Hidradenitis suppurativa

Hidradenitis suppurativa can cause chronic acne-like inflammatory lesions and pustules that leave scars. It occurs in the armpits and may also affect the genital and anal areas and the skin beneath the breasts.

A characteristic finding is a double-headed comedone: a blackhead with two skin openings connected beneath the surface. This may appear years before other symptoms. Lesions can progress to wounds in the armpits, and firm, cord-like scars may be felt within the skin.

Causes and medical assessment

Hidradenitis suppurativa, or HS, is a chronic inflammatory skin disease that differs from ordinary acne. It involves blocked hair follicles and inflammation, but its exact cause is not yet known. Hormones may play a role and it often begins after puberty. Age or menopause should not be used to rule it out. Recurrent painful lumps or scarring in the armpits or groin should be assessed by a dermatologist.
